How to apply

Information on this page describe the Multi Purpose Taxi Program (MPTP) application process and includes for applications forms for applicants and their doctors.

Applicants

To become an MPTP member, both you and your doctor have to fill in an application form. Once the form has been filled in, send it to the Victorian Taxi Directorate (VTD). The address is on the form.

You can download the membership application form and an information booklet. You can download them as PDF files or as Rich Text Format (RTF) files. You can also contact the VTD to have them sent to you in hard copy.

A Change of Circumstance form is also available for you to download and complete in case you need to update your details or report a damaged/lost MPTP card. This form is available in PDF (313 KB, 4pp.) and RTF (18KB, 5pp.) formats.

Doctors

Please carefully assess your patient against the criteria in the forms above.

Please complete the online form and submit electronically. For further information please contact the VTD on 1800 638 802.

You are not required to complete the paper-based part of the form – this is your patient's responsibility.

Please note: The MPTP member must not travel in a wheelchair accessible taxi when using a tilting or reclining mobile chairs as a mobility aid. These include high care chairs/beds, Princess, Duchess or Tub chairs.  

Assessment

The VTD will decide if you can become a member and may ask you or your doctor for more information to help it decide.

The VTD may ask you to see an independent doctor or have medical tests to confirm the information you gave in your application. Normally you will be asked to pay for this.
