Taxi incident notification form

Victoria Police has established a new incident reporting system for the taxi industry.

By filling out the taxi incident notification form (PDF, 100 KB, 1 p.) you will assist Victoria Police determine trends and allocate law enforcement resources to tackle safety issues in the taxi industry.

You can also access a copy of the form from your depot, VTD Taxi Inspection Centre at Melbourne Airport and the VTD Customer Service Centre on Level 23, 80 Collins St, Melbourne.

Important: drivers are still required to report instances of crime directly to a police officer or the nearest police station for the incident to be investigated.

In emergency situations, drivers should still dial '000'.

Please note that this form should only be completed by a taxi driver or operator. Taxi passengers wishing to lodge a complaint about their taxi trip need to submit a feedback or complaints form.
